PhilHealth allows transactions ok'd by its members

 

FOLLOWING circuitous routes beneficiaries of the Philippine Health Insurance Corporation (PhilHealth) take to transact business with members absent, the social service provider has finally approved authorized persons to personally transact business at PhilHealth.

In their order of priority, authorized to sign any PhilHealth document such as Claim Form1, M1B, M2 and other related documents, in behalf of the members are the legitimate spouse, mature child of member, parents (biological/adoptive/step-parent) matured brothers or Sisters and other authorized representatives.

PhilHealth however requires the member to issue an authorization letter stating the reason for the signing in his behalf. The authorization letter must contain the full name, complete address and contact number of the authorized representative. In addition, the representative shall submit a photocopy of his identification card, authorities bared.

In the case of an OFW who shall file an application for membership, PhilHealth will require a photocopy of any document providing that the worker is an active OFW, such as working visa, working permit, or employment contract.
